About D&S Homez

 

D&S HOMEZ LLC, is a Licensed Adult Family Home under Wisconsin Administrative Code DHS 88. We provide a home-like environment servicing the following clients groups: Developmentally Disabled, Emotionally Disturbed and Mentally ILL, Advanced age and Dementia/Alhizemers. We are pet friendly. It is located at 3338 N 2nd street, Milwaukee, Wisconsin 53212. D & S HOMEZ is a single-family home.The home has 4 Private bedrooms, 1 full bathroom and two half restrooms, living room, and laundry facilities. We are licensed to serve up to 4 residents. The house is not equipped for residents in wheelchairs or for those who walk only with the use of crutches or a cane. Home is not handicap accessible.

 

Admission Policy: Residents considered for admission to D&S HOMEZ must be programmatically compatible with each other. Similar likes, dislikes, and physical abilities are taken into consideration during the initial intake assessment. D&S HOMEZ welcomes individuals who exhibit challenging behaviors, but may exclude persons who are actively suicidal, who are destructive of property, unless an Individual Service Plan identifies the areas of risk and serves as a reasonable plan to minimize these risks. Initial referral and assessment information must be at D&S HOMEZ upon admission of resident. Within 90 days prior to admission, each resident must be examined by a physician or registered nurse and found to be free of communicable disease. Documentation of this examination is required prior to admission. Letters of Guardianship, Protective Placement of any applicable legal documents must be at D&S HOMEZ upon admission of a resident.

 

Program Goals: The overall goal of D&S HOMEZ is to maximize the residents’ cognitive, affective, physical health, and psychomotor skills in order to help them obtain their highest possible level of functioning. This will be accomplished through attitudes of hopefulness, a structured, yet warm environment, individual treatment plans with realistic goals, and the opportunity for group living and social interaction. Imperative to the program is an emphasis on community integration to the greatest extent possible for each resident. Examples of such integration are religious activities, cultural events, and social interaction through community events and activities. Each resident will have a goal oriented Individual Service Plan that will be D&S HOMEZ Program Statement discussed and reviewed with case managers, guardians, families, and others involved in their lives.

 

Services: Services provided by the home and covered under the daily rate will include: ● Supervision by trained staff. ● Information and referral services. ● Leisure time support services. ● Assistance with independent living skills. ● Health monitoring services and nursing consultation. ● Meals and nutritious snacks and accommodation for special diets. ● Provide transportation for residents to participate in community activities; and arrange for transportation to/ for work, medical appointments, education and religious services. ● Individualized service planning. ● Assistance in communication with family, friends, and professionals. ● Community activities. These services are provided to support successful community inclusion, to maximum independence, to promote individual choices, and to enhance the quality of life for adult residents of all ages.

 

Specific Program Goals Include:

●  Assisting and training residents in personal care activities such as dressing, grooming, and bathing.

●  Assisting and training residents in daily living skills such as meal planning and preparation, laundry, and household chores.

●  Assisting residents in appropriate socialization, including productive and independent leisure time, one-on-one interaction, group activities, as well as community work or day programs.

●  Assisting residents in understanding and expressing their emotions and feelings, focusing on building good self-esteem and sense of self-worth. D&S HOMEZ Program Statement

●  Encouraging and helping residents in maintaining involvement with family members and other significant people in their lives.

●  Accurately assessing and responding to the unique and changing needs of each resident.

 

Staffing Level and Training: D&S HOMEZ LLC will have at least one staff on duty at ALL times to ensure our program goals for each resident are met. The ratio of staff to residents shall be adequate to meet the needs of the residents as defined in their assessments and current Individual Service Plans. Our staff consists of trained Resident Assistants with CBRF certifications. Staff are trained in First Aid, Medication Monitoring, Fire Safety and Standard Precautions and additional 8 hours of training is offered throughout the year for each staff member within the client groups served. Transportation: D&S HOMEZ LLC will provide transportation to appointments, outings, etc. and although we primarily use company vehicles, we do have the right to transfer transportation duties to service providers with a valid driver’s license and proof of liability insurance.

 

Safety Issues: D&S HOMEZ LLC, has emergency plans in place in case of any natural disaster, emergency or safety issue. Please refer to emergency evacuation plans, policy and procedures and or floor plan.

 

Health Monitoring Services: Health monitoring services are provided by your own physicians and nurses, or those within the community, who ensure that each resident receives an initial health screening assessment and at minimum, annual examinations. Staff will ensure that residents receive initial, follow-up and D&S HOMEZ Program Statement emergency health monitoring services. Staff will work closely with doctors and healthcare providers to ensure that residents are attending appointments, informed of appointment dates and transported to appointments.

 

Medication Monitoring: Medications will be self-administered and monitored by trained staff. Medication administration by authorized staff will occur only when ordered by a physician.

 

Leisure Time Services: Leisure time services are designed to maximize meaningful and productive activities in the community as well as at home. Residents have the opportunity to structure their leisure time individually and through group consensus. Activities may vary from a variety of day programming options in the community, to at home activities such as music, cooking, crafting, exercise and visiting the library, local shopping malls, restaurants, and nature hikes. As well as, picnics in several area parks, sporting events, special community events. Special events and vacations are scheduled throughout the year.
